You can get a copy of an immunization or vaccine record for yourself or your child using My Vaccine Record. You will need a valid form of identification.
Printouts from My Vaccine Record are official reports that you can use for school, college, or camp enrollment. You can also show it as proof of COVID-19 vaccination.
Records You Can Access
My Vaccine Record only has records for immunizations that were reported to the Department of Health and Mental Hygiene (DOHMH) by NYC health care providers.
NYC health care providers must report:
- All immunizations given to children 18 and under in NYC
- All COVID-19 vaccine doses given in NYC
Most records are for people born in NYC after 1995. Only some vaccinations given to adults ages 19 and older are reported, so records may not exist or be incomplete.
Who Can Access Records
You can access your own immunization record if you are 18 and older.
To access your child’s records, you must be listed on the child’s birth certificate or immunization record as the child’s:
- Parent or guardian
- Next of kin
- Primary contact
