A permit is required for the use of a therapeutic medical linear accelerator (LINAC) or radiation therapy unit capable of operating at 500 kV (photons) and/or 500 keV (electrons) or higher.
Permits are issued to radiation therapy providers, including:
- Hospitals
- Private physicians
- Veterinary clinics
- Veterinarians
Fees
The fee for a new facility permit is $600 for 5 years.
The fee for every additional 5 years is $600.
The cost to amend a permit is $235.
Expiration Date
The permit expires after 5 years.
