Case managers meet with homebound or frail adults in their home. This is for adults age 60 and older.
A person is considered homebound if:
- They are unable to leave home without help
- This is due to a physical or mental illness
Case managers can help with:
- Supportive contact
- Authorization for home care
- Home delivered meals
- Adult care services
You can receive a referral to a local case management agency if you are:
- Homebound or frail
- Age 60 and older

SCRIE Application Assistance
Case Management Agencies can help homebound older adults who cannot go to an Older Adult Center for case assistance, and who do not have anyone who can come to their home to help them, to complete an application for the Senior Citizen Rent Increase Exemption (SCRIE).
These agencies can also assist with SCRIE renewal applications.
