You can request a copy of a Police Crash Report within 30 days of the collision online, by mail, or in person.

By Mail

Mail a "Request for Copy of Collision Record (PD301-165)" to the Administrative Quality of Life Officer of the precinct where the collision occurred. The form is available at any precinct.

Include a photocopy of a valid form of picture identification (for example, a driver's license) and a self-addressed stamped envelope.

In Person

Go to the precinct where the collision occurred with a valid form of picture identification (for example, a driver's license or passport).

You can request a Police Crash Report from the State if the accident occurred over 30 days ago. To request a report, you must complete and mail form MV-198C (Request for Copy of Crash Report) to the State.
