Each year, the Department of Health and Mental Hygiene (DOHMH) conducts Health Survey 50. It is a confidential survey to learn how current and former New York City residents seek out and access health care and social services.
If you are chosen to participate, you will receive a $75 gift card.
DOHMH chooses participants at random. You can’t volunteer.
